I agree that this is a very good idea but I am also concerned about safety and security of the information. There could be a similar system of secure, yet sensitive information such as what the government and the top software companies have.

That is a great idea, but my only concern would be privacy. If we post our personal health info, health log on the web, we will become vulnerable to multiple different dirty techniques used on the web to collect personal data, solicit with advertisement, identity theft and much much more. But the positive effects are also clear, medical diagnostics online, cheaper or for free medical consultation and of course this technology will take big piece of business from rich doctors who charge for sinmple consultation $200 to $500. The idea is really great but it has to be stable and secure. and if the competition will be the same as among telephone manufacturers (VTech, Nokia, Motorola), I`'m pretty sure it will be a great product

Great, thoughtful article. Clay Christensen's challenge to HC may succeed, in part, if your PHIN is properly executed i.e., truly consumer-centric. This means a trusting partnership to support their ownership of PHI. The same type of relationship seen when HC services are rendered to people who are sick and focusing on getting better, not collecting their PHI. Ownership w/o an established trusting support network (defined by the consumer) will only overwhelm them and fail. So, as you indicate in your "Hello..." example, this should be "spoken" by the trusted representtive only after chosen by the consumer.

PHIN is not a good name for the Personal Health Information Network, PHIN is already taken my the CDC for the Public Health Information Network. ONC and AHIC have developed standards for eHealth vocabulary terms, I think that PHR is the proper reference (Personal Health Record).

Google Health, Health Vault or Dossia all seem to be still more top-down, take-it-or-leave-it solutions from the patients' point-of-view. Where is my ability to download MY health records to MY flash drive? Where is the ability for me to install an application to MY computer so that I can review MY medical records for errors of date, fact, etc without being forced to go on-line? After all, most of the world (even much of rural USA) still does not have easily-available, affordable internet access.

Think this will cause some confusion in the federal government and industry.

Vicne, love the article and it is something my colleagues and I have been discussing for many years. What will be interesting, though breifly discussed, will be the monetization of PHI combined with the adoption of open values and market forces. My prediction is that we will see tools, such as EMRs in doctor's offices, provided free as a service by the likes of GH in order to accumulate PHI data in mass. The effects will rapidly boost EMR adoption and in turn rapidly accelerate the PHIN., dramatically changing the current information landscape. Look no futher than Google's relationship with Practice Fusion to obtain a glimpse into a much broader strategy.

As in life, birth announcements are usually welcomed events. Some represent the result of hopes and collaborative planning. Others do not. In either case, survival is a much larger--and longer-- challenge than conception. With typical vision, Vince, you have forecast a brave new world of opportunity, while recognizing there are many existing forces that would not like to see this child survive. It will be interesting, and sometimes fun, to participate in the struggles ahead.

There is little potential resistance to Google’s request to hand over the data, because the only requests should be to organizations that have already agreed to participate in Google Health, and have signed NDA's and partner with Google in the program.
